Tour Agreement:
I recognize that MogelWerf LLC, of Royal Oak, Michigan, dba MogelWerf
is acting solely as agent and is responsible only for making the arrangements
for all accommodations or services which constitute the trip, except to
the extent that accommodations or services cannot be supplied due to reasons
beyond the control of that agent. I recognize that MogelWerf is not in
any way liable for personal injury, property damage or any loss arising
out of the act, omission, or negligence of any direct air carrier, motor
coach company, hotel, property management company, ski area, or any person
rendering any services or accommodations in conjunction with the tour.
Furthermore MogelWerf is not in any way responsible for any injury, damage
or loss due to reason of theft, accident, mechanical breakdown, government
action, weather, or any reason beyond their control. Any agreement to
accept this proposal shall be considered to be entered into in Michigan
and the laws of the state of Michigan shall apply hereto. I am familiar
with and hereby agree to the cancellation policy as printed in the tour
information brochure. I am authorized to make charges to the credit card
provided and hereby authorize MogelWerf to charge the credit card provided
for payment of my tour. I also authorize MogelWerf to charge the credit
card provided for any damage charges assessed against me.
I also understand/certify
that:
- It is
my responsibility to secure all transportation for this trip.
- I am 18
years of age.* If not, then a signed permission slip must be
provided from your legal guardian.
- If I cancel
my trip, only 25% refund is available after November 1, 2004, unless
I find someone to take my place.
